How to Verify a Provider Independently
1. Ask the provider directly
- What is the legal name of the organization operating the program?
- What license does it hold, from which state agency, and under what license number?
- Which services are provided on site, and which are referred elsewhere?
- What is the full cost, and what does it include?
2. Check the state
New Jersey publishes health facility licensing information through the Department of Health, and individual professional licenses can be verified through the Division of Consumer Affairs. Confirming what a provider tells you against a state source is a reasonable step before committing to anything.
3. Confirm the location and the service
Ask where care is actually delivered, since a phone number or web page may be operated somewhere other than the treatment site. Ask whether the address you found is a clinical location, an office, or a call center.
